One of the most common misconceptions about hair fall is that it's mostly genetic or stress-related. While both of those matter, a significant proportion of hair loss in India — particularly among women — is directly caused by nutritional deficiencies that can be identified with a blood test and corrected with appropriate treatment.
The nutritional causes are especially important because they're reversible. Genetic androgenetic alopecia cannot be undone. Iron deficiency causing hair fall can be corrected, and hair regrowth typically follows.
This guide covers the specific deficiencies known to cause or worsen hair fall, how to test for them, and what to do.
Iron is not a vitamin, but it's the most significant nutritional cause of hair fall in Indian women and deserves first position.
Why iron matters for hair: hair follicles are among the most metabolically active cells in the body, requiring constant oxygen delivery. Oxygen is carried by haemoglobin, which needs iron. Even mild iron insufficiency reduces the oxygen supply to follicle cells, causing them to exit the growth (anagen) phase early and enter the resting (telogen) phase.
The ferritin problem: standard blood tests check haemoglobin. Many women have haemoglobin in the "normal" range while their ferritin (the iron storage protein) is very low. Hair loss research consistently shows that ferritin below 30–40 ng/mL causes hair fall even when haemoglobin is normal.
The conventional laboratory "normal" for ferritin is above 12–15 ng/mL — which is far too low for hair health. For hair growth, ferritin above 70 ng/mL is the level associated with optimal follicle function.
Testing: ask specifically for serum ferritin, not just a haemoglobin or complete blood count.
Correction: iron-rich foods (rajma, masoor dal, ragi, drumstick leaves, eggs, red meat for non-vegetarians) with vitamin C to enhance absorption. Iron supplements (ferrous bisglycinate is most gentle) when ferritin is below 30 ng/mL. Results on hair typically take 3–6 months to become visible after ferritin normalises.
Vitamin D receptors are present in hair follicle cells, and vitamin D directly regulates the hair growth cycle. Research has specifically linked vitamin D deficiency to:
India has a paradoxical vitamin D deficiency epidemic despite being one of the sunniest countries. 70–80% of urban Indians have insufficient vitamin D levels. The reasons: indoor work, full clothing coverage, dark skin requiring longer sun exposure for equivalent synthesis, and the low UV index in North India during winter months.
Testing: 25-OH vitamin D blood test. Optimal level for hair health: above 30 ng/mL. Many Indians test below 20 ng/mL (deficient).
Correction:
Hair improvement after correcting vitamin D deficiency typically begins at 3–4 months.
Vitamin B12 is essential for the production of red blood cells that supply oxygen to hair follicles, and for DNA synthesis in the rapidly dividing cells of the hair matrix. Deficiency causes both anaemia (which impairs oxygen delivery) and direct impairment of follicle cell division.
B12 deficiency is very common in India — affecting up to 60–70% of vegetarians in some studies. The reasons: B12 is found almost exclusively in animal products (meat, fish, eggs, dairy). Even people who consume dairy may be marginally deficient because the B12 content of dairy is lower and less bioavailable than that of meat.
Additionally, long-term metformin use (extremely common in India for diabetes and PCOD) reduces B12 absorption in the gut — all metformin users should have B12 checked annually.
Symptoms beyond hair fall: fatigue, weakness, tingling in hands or feet, mouth ulcers, memory difficulties, elevated homocysteine.
Testing: serum B12. Optimal: above 300 pg/mL. Hair fall risk increases below 200 pg/mL.
Correction:
Hair recovery after B12 correction takes 3–6 months.
Zinc plays a fundamental role in DNA synthesis, protein synthesis, and the production of hair-specific proteins. It's required for normal follicle cell cycling and the maintenance of sebaceous glands.
Both deficiency and excess cause hair fall — making zinc one where the goal is adequacy, not supplementation beyond the recommended range.
Signs of zinc deficiency beyond hair fall: white spots on nails, delayed wound healing, frequent infections, loss of taste or smell, skin problems.
Who is at risk: people on predominantly plant-based diets (zinc from plants is less absorbable than from meat because of phytates), those with gut absorption problems, and people who have been dieting aggressively.
Testing: serum zinc level. May not catch marginal deficiency — clinical assessment alongside dietary history is also useful.
Correction:
Biotin is the nutrient most aggressively marketed for hair fall. The marketing is not matched by the evidence.
Genuine biotin deficiency causing hair fall exists — but it's genuinely rare in people eating a normal diet. Biotin is found in eggs (yolk particularly), peanuts, almonds, sunflower seeds, and many other common foods. The daily requirement is small (30 mcg for adults) and easily met.
Situations where biotin deficiency is genuine:
What the evidence shows for biotin supplementation without deficiency: there is no quality evidence that supplementing biotin beyond dietary needs improves hair growth in people who are not deficient. Multiple published case reports of "biotin supplementation improving hair" involved people who were actually deficient.
The practical implication: don't spend money on high-dose biotin supplements unless you have confirmed deficiency or one of the specific risk situations above. Invest those resources in iron, vitamin D, and B12 testing instead.
Niacin is required for cellular energy production in hair follicle cells and has vasodilatory effects on scalp blood vessels (improving blood supply). Severe niacin deficiency (pellagra) causes hair loss among other symptoms.
Subclinical niacin insufficiency contributing to hair fall is less well-characterised but plausible.
Dietary sources: peanuts, whole grains, eggs, poultry. Pellagra is rare in India today, but people on very restricted diets may be marginally insufficient.
Selenium is essential for antioxidant protection of follicle cells and thyroid hormone metabolism (thyroid dysfunction is a major hair fall cause, and selenium is required for healthy thyroid function).
Both deficiency (rare in Indian diets with their varied grain content) and excess (from oversupplementation) cause hair loss.
Don't supplement selenium without testing. If taking a multivitamin-mineral supplement, ensure selenium content is within the RDA (55 mcg for adults; don't exceed 400 mcg).
Folate is required for the DNA replication happening in rapidly dividing hair matrix cells. Deficiency impairs this division, reducing hair production. More commonly known for its role in pregnancy (neural tube defect prevention), but relevant to hair year-round.
